Health Centers' Behavioral Health clinics in Oregon City moved to a new permanent home to serve you better!
Our services will soon move to a newly renovated permanent location on Lake Road in Milwaukie where we can provide you with enhanced services. In the meantime, we are temporarily located to the county’s Development Services Building (DSB) on Beavercreek Road in Oregon City.
Sandy Clinic Grand Opening
Clackamas County’s Sandy Health Center will soon be offering the same great primary, dental and behavioral health care in a beautiful new location.
Health Information Management (HIM)
We provide patients and clients access to electronic and paper copies of their medical records; they can also send records to others at the client/patient’s request.
Fees for Providing Copies of Patient/Client Medical Records
Per ORS 192.521:

Behavioral Health Centers
We believe that individuals can, and do, achieve wellness and fullness of life even when dealing with significant mental health and addictions concerns.
We offer a variety of outpatient counseling and treatment services for mental health, drug and alcohol, and co-occurring issues for individuals of any age. These services are designed to create hope, explore possibilities, and help individuals and families to develop new strategies to heal and grow.
