How to Get a Birth Certificate

If you need a birth certificate for a baby born less than six months ago in Clackamas County, please follow the steps outlined below. If the event was more than six months ago or in a different county, order through the Oregon Health Authority. For more information, please contact the Vital Records office at 503-655-8406.

Visit the Vital Records office at 2051 Kaen Rd #367 Oregon City, OR 97045 to complete birth certificate order form. Note: You must provide a copy of your current photo ID. See list of valid forms of ID here.
